Medical conditions DVLA need to know about
You can download the appropriate medical questionnaire in order to notify the Driver and Vehicle Licensing Agency (DVLA) about your medical condition or disability. The medical questionnaire inhcludes a consent form that allows the DVLA to carry out medical enquires into your fitness to drive.
